The Three Element Model (3EM) is a method to decrease diabetes related health disparities by transforming diabetes care from silos of exam-room centered medical practice and public health education to the three-component, integrated model of transformed practices supporting empowered/ engaged patients in family/community context with rapid-cycle outcome-improvement surveillance feedback loops. Practice Transformation is demonstrated by care management, panel management and outcome management. Community engagement allows patients to be treated as free range humans and supports and engages them in the community where they live work, play and pray. Rapid cycle data feedback loops promotes useful data collection and analysis to drive improvements and prove that they are effective.
